Research suggests these tools can offer real benefits for mild-to-moderate symptoms, but safety experts warn they are not a replacement for professional care -- and in crisis situations, they may fall dangerously short. This guide breaks down how AI therapy chatbots work, what the evidence shows, and the key safety rules every user should know.
What Are AI Mental Health Chatbots?
An AI mental health chatbot is a software application designed to provide emotional support, psychoeducation, and coping strategies through conversation. Unlike a licensed therapist, these tools are available 24 hours a day, seven days a week, and typically cost far less than traditional therapy -- or nothing at all.
Several AI therapy chatbot platforms have gained wide recognition. Woebot, developed by Stanford psychologists, delivered structured cognitive behavioral therapy (CBT) exercises through a chat interface and built one of the strongest clinical evidence bases in this space. Wysa, a penguin-themed app, combines CBT, dialectical behavior therapy (DBT), and mindfulness tools and has earned FDA Breakthrough Device status for its potential mental health impact. More general-purpose large language model (LLM) tools -- including ChatGPT, Claude, and Gemini -- are also increasingly used for informal emotional support, though they are not purpose-built or clinically validated for mental health care.1
It is important to understand the distinction between purpose-built mental health AI apps and general-purpose chatbots. Purpose-built tools like Wysa are designed with clinical guardrails, crisis pathways, and evidence-based frameworks. General LLM chatbots are not regulated as medical devices and were not designed for therapeutic use.2 This distinction matters significantly when evaluating safety and appropriate use.
How They Work
Most dedicated AI mental health chatbots are built on CBT frameworks -- the gold-standard talk therapy for conditions like depression and anxiety. CBT helps users identify and reframe unhelpful thought patterns. Within a chatbot, this plays out through guided exercises, mood-tracking check-ins, and structured journaling prompts delivered conversationally.
Technically, these apps use natural language processing (NLP) to interpret what you write and select appropriate therapeutic responses. Earlier tools like Woebot used pattern-matching to select from a library of therapist-approved responses, ensuring consistency and reducing the risk of off-script or harmful replies. More recent tools powered by LLMs generate responses dynamically, which can feel more natural but introduces greater variability and unpredictability.3
Many apps include additional features such as mood logs, breathing exercises, sleep hygiene tips, and psychoeducational content. Some, like Wysa, apply sentiment analysis and intent recognition to gauge emotional tone and respond accordingly. A small subset of AI mental health apps have pursued formal regulatory pathways: Wysa holds FDA Breakthrough Device designation, and some digital therapeutic platforms have pursued FDA clearance as prescription digital therapeutics (PDTs).
What Research Shows About Effectiveness
Clinical research on AI mental health support tools has grown significantly, though important limitations remain. A 2024 systematic review published in PMC examined studies on Woebot, Wysa, and Youper and found large improvements in depression and anxiety symptoms across all three platforms, with users reporting high satisfaction rates and a meaningful sense of therapeutic alliance -- the feeling of connection with the tool.3
Woebot's early landmark study, a randomized controlled trial, found that two weeks of use was more effective at reducing depression symptoms than a control group given a self-help e-book. Users averaged approximately 12 sessions over that period. Wysa has been supported by more than 30 peer-reviewed studies, with participants showing notable reductions on standardized PHQ-9 depression and GAD-7 anxiety symptoms scales compared to baseline.3
However, meta-analyses of the broader field indicate that effect sizes tend to be small and may not be sustained over time. A 2025 World Psychiatry systematic review charting the evolution of AI chatbots noted that only 16% of studies involving large language model-based chatbots underwent clinical efficacy testing, with the majority still in early validation stages.4 Researchers consistently note that these tools are not equivalent to working with a human therapist, and that long-term evidence is still lacking.
Benefits: Why People Are Using Them
Despite the evidence limitations, there are real and meaningful reasons why people turn to AI mental health tools.
Accessibility and availability. Mental health care faces a severe shortage of providers in many regions. Chatbots are available immediately, around the clock, without waitlists. For someone experiencing anxiety symptoms at 2 a.m., a chatbot may be the only resource available in that moment.
Lower cost. Many chatbot apps are free or low-cost compared to therapy, which can run $100 to $300 per session without insurance coverage.
Reduced stigma. Research consistently shows that some people are more willing to disclose sensitive mental health information to an AI than to another human, due to reduced fear of judgment. This lower barrier to entry may help people take a first step toward addressing their mental health.
Supplemental support between sessions. For people already in therapy, chatbot tools can serve as a useful bridge -- reinforcing skills, tracking moods, and providing coping strategies between appointments.
Mild-to-moderate symptoms. Evidence suggests AI mental health support is most appropriate for people experiencing mild-to-moderate depression symptoms or anxiety, rather than severe or complex presentations.
Risks and Limitations
The risks associated with AI mental health chatbots are significant and should not be minimized. Understanding these AI health chatbot risks is essential before relying on any of these tools.
Crisis situations. This is the most serious concern. A 2025 simulation study found that when AI chatbots were given prompts simulating people experiencing suicidal thoughts, delusions, or mania, some chatbots validated delusional thinking and encouraged dangerous behavior. In a separate study, AI chatbots actively endorsed harmful proposals in 19 out of 60 (32%) simulated adolescent crisis interactions.5 Purpose-built apps like Wysa include crisis pathways that refer users to emergency services, but general LLM chatbots do not have reliable safeguards for these situations.
ECRI's top health hazard for 2026. The ECRI Institute -- an independent patient safety organization -- ranked the misuse of AI chatbots in healthcare as the number one health technology hazard for 2026. More than 40 million people daily use ChatGPT for health information, yet these tools are not regulated as medical devices or validated for healthcare use. ECRI specifically flagged the potential for chatbots to provide false or misleading information that could result in significant patient harm.2
No clinical oversight or accountability. Licensed therapists are governed by professional boards and can be held liable for malpractice. When an AI chatbot provides harmful guidance, there is currently no established regulatory framework for accountability.4
Privacy concerns. AI mental health apps collect sensitive personal information. Privacy policies vary widely, and it is not always clear how data is stored, shared, or used to train AI models. Reviewing an app's privacy policy before use is an essential step.
Risk of delaying real treatment. Relying on a chatbot for symptoms that require professional care -- including severe depression, trauma, psychosis, or suicidal ideation -- may delay someone from getting help that could be life-saving. The dangers of self-diagnosing with AI extend into the mental health space, where overreliance on AI feedback can create a false sense of being "handled."
Algorithmic bias. Research has found that some chatbots display increased stigma toward certain mental health conditions, including alcohol dependence and schizophrenia, compared to conditions like depression. This bias may affect the quality and appropriateness of responses for some users.5
How to Use AI Mental Health Tools Safely
If you choose to use an AI mental health chatbot, a few key principles can help you do so more safely.
Use it as a supplement, not a replacement. AI tools work best alongside professional care, not instead of it. If you are in therapy, discuss any chatbot use with your therapist. If you are not in therapy, a chatbot should not substitute for a proper mental health evaluation.
Choose purpose-built apps over general chatbots. Apps designed specifically for mental health support -- with clinical frameworks, crisis pathways, and evidence backing -- are safer choices than general-purpose LLMs for mental health support specifically.
Review privacy policies before sharing sensitive information. Understand what data the app collects, how it is stored, and whether it is shared with third parties or used for AI training.
Know the warning signs that a chatbot is not enough. If you are experiencing thoughts of self-harm or suicide, hearing voices, feeling disconnected from reality, or are in acute distress, stop using the chatbot and contact a human immediately. In the United States, you can call or text 988 (the Suicide and Crisis Lifeline) at any time.
Verify information with a healthcare provider. Just as with any digital health tool, do not make decisions about medications, diagnoses, or treatments based solely on chatbot guidance. Always verify important health information with a qualified provider.
Watch for red flags. Be cautious if a chatbot validates grandiose or delusional thinking, discourages you from seeking professional help, or provides specific guidance about medications. These are signs the tool is operating outside safe limits.

You should seek care from a qualified mental health professional -- rather than relying on an AI mental health chatbot -- if you experience any of the following:
Persistent feelings of sadness, hopelessness, or emptiness lasting more than two weeks
Thoughts of self-harm, suicide, or harming others
Significant changes in sleep, appetite, or ability to function at work or in relationships
Hearing voices or experiencing beliefs that feel disconnected from reality
Panic attacks or anxiety so severe it interferes with daily life
Substance use that feels out of control
A mental health condition that has required medication or hospitalization in the past
